- Church Name
- Team Rosters with first and last names plus team names for the year
- Total number of $5 lunches (include teens, coaches, parents, siblings, friends)
- List quiz equipment that you have available and bring it with you to quizzing with all parts labeled (ask Valerie at check-in where to take your items)
- Coaches names and cell phone numbers
- Pay team fees and lunch fees at check-in
- 2-5 quizzers $10
- 1-3 quizzers $5 IF we ask you to combine with another team
- Lunch fee $5 per person
- Only church checks accepted made payable to NEI NYI
